Your prescription information allows us to custom make your lenses in your exact prescription. Please note that correct plus (+) and minus (-) signs should be indicated for all sphere and cylinder powers. The pupillary distance is needed to fabricate your lenses. Sometimes you may not find it on your written prescription. If your PD has two numbers (eg 64/61) use the first, larger number for the number to select from our form. Your eye care provider should be able to provide you with this number. Many of our customers have obtained their PD by calling the last place that they purchased their glasses. If you are unable to read any of the numbers on your written Prescription do what we do. Call your eyecare professional and ask them to interpret. Remember your legal right to your eyeglass prescription has been confirmed by recent federal rulings. If your prescription contains a prism correction we would prefer to see a written copy of your prescription by Fax. The fax number is 715-423-3330.
